Please note recent changes to our policies with regards to:

Patient Responsibilities and Commitment for Participating in Group Appointment Series

• The group medicine series requires attendance of every appointment in the series for maximal education and physiological benefit.
• If you miss an appointment in the series, you understand that you will incur a $100 cancellation fee per missed appointment.
• The 1 week cancellation policy applies to the entire series, not per appointment.
• If you cancel the series outside of the 1 week cancellation policy (or 7 business days), you will incur the $100 missed appointment fee for each appointment in the series.
• Ex: if it's a 4 week series you will owe $400, if it's an 8 week series you will owe $800
• If class begins on Monday the 7th, you need to cancel the class before Monday the 1st since the policy applies to 7 business days.
